Chain of Custody

 

Accurate and complete Chain of Custody (COC) records after sample collection are vital to ensure that sample data is legally defensible.   Each custody transfer must be documented & signed on the COC from sampler to the laboratory. Ink Only.

COC’s are required by law for every test we run for our clients. They include the following information: sample identification, location, date and time of collection, collector’s name and signature, sample matrix type, analysis requested and any comments regarding the sample.

The COC serves as record-able means for clients to communicate sample information and requested analyses to the laboratory. Samples must be labeled in conjunction with the sample identification on the COC.

Whenever revisions are made to a COC please use this practice; Cross out disregarded information with one line through the text, then date and sign with your initials.
